首先顶格写称谓，即向谁请假，注意应加上其职务，以示尊重。

第二行空两格开始写正文
首先写明请假事由，其次写明请假时间，最后加上请假习惯用语“请批准”、“请予批准”等。请假条内容较少的，不用分段。
语言应朴实，简单，不能作无谓的修饰，把事情说得清楚简明就好。
